Brand Application
Application for Livestock Brand Record. Each brand is registered for a five-year period (original registrations are for four full years after the following April 1) with brands expiring on April 1. Each calendar year ending in the number "9" or "4" has a larger number of brands for renewal than for other years. The brand registration fee and the renewal fee are each $45.00, the fee for transfer of ownership is $15.00. Trichomoniasis Female Import Affidavit

Trichmoniasis Herd Management Plan

A feedlot is defined as a livestock feedlot or feed yard with more than 1,000 head of livestock at one time during the license year, or any other livestock feedlot whose operator elects to come under the act. Domesticated Cervid Permit
Requires anyone who raises or harbors domesticated deer to have a permit issued by the department. Deer includes all members of the family cervidae (deer, elk, moose, caribou and related species).

Livestock Dealer Registration
Requires the licensing of livestock dealers or order buyers by the department. This includes any person engaged in the business of buying or selling livestock in commerce, either on that person's own account or as the employee or agent of the seller or purchaser, or any person engaged in the business of buying or selling livestock in commerce on a commission basis and shall include any person who buys or sells livestock with the use of a video.

Requires premises that sell livestock on a consignment basis to be licensed with the department. These requirements include the traditional livestock market as well as occasional sales that provide for the public sale of livestock.

Swine PIN Tags
To order swine Premises Identification Number (PIN) tags, first establish if your farm already has an existing PIN assigned to it by calling the KDA office at (785) 564-6601. If you do not have an existing PIN, use the Kansas PIN Registration Form to provide KDA with the information they will need to create a new PIN for your premises. To order PIN tags, visit the Pork Checkoff's website for PIN Tag resources. There you will find a list of approved tag suppliers and contact information.

Animal Shelter or Pound License Application - a facility which is used or designed for use to house, contain, impound or harbor any seized stray, homeless, relinquished or abandoned animal or a person who acts as an animal rescuer, or who collects and cares for unwanted animals or offers them for adoption. Animal shelter also includes a facility of an individual or organization, profit or nonprofit, maintaining 20 or more dogs or cats or both, for the purpose of collecting, accumulating, amassing or maintaining the animals or offering the animals for adoption.

Rescue Network License Application- the premises of a rescue network manager and all pet animal foster homes organized under that rescue network manager that provide temporary care for one or more dogs or cats not owned by an animal shelter that maintains a central facility for keeping animals.  A rescue network manager is a person designated by the rescue network that is responsible for approving membership for each foster home, intake of all animals, maintaining all records, and ensuring compliance of each foster home that belongs to the rescue network.

Pet Shop License Application -  any premises where there are sold, or offered or maintained for sale at retail and not for resale to another:  (A) Any dogs or cats, or both; or (B) any other animals except those produced and raised  on such premises and are sold, or offered or maintained for sale, by a person who resides on such premises.  Pet shop does not include:  (A) Any pound or animal shelter; (B) any premises where only fish are sold, or offered or maintained for sale; or (C) any animal distributor premises, hobby breeder premises, retail breeder premises or animal breeder premises.
